What Are Complex Patterns Combining Deficiency and Excess?

In TCM, complex patterns involving both deficiency and excess describe conditions where the body experiences a mix of insufficient vital substances (such as Qi, Blood, Yin, or Yang) and
excess conditions (such as stagnation, accumulation, or heat). These patterns are complex because they require addressing both the deficiencies and excesses simultaneously to manage
symptoms effectively.
How Do These Patterns Relate to Pain?
Pain in TCM can be a manifestation of both deficient and excessive conditions. Here's how complex patterns combining deficiency and excess might present in terms of pain:


1. Qi Deficiency with Blood Stagnation:
○ Pain Characteristics: Pain may be dull and achy due to Qi deficiency, combined with sharp or throbbing pain from Blood stagnation. This can result in localized
discomfort that feels worse with physical activity or emotional stress.
○ Common Areas Affected: Often in areas where Qi and Blood flow are impaired, such as joints, muscles, or the lower abdomen.
2. Yang Deficiency with Damp-Cold:
○ Pain Characteristics: Pain tends to be dull and feels cold, with associated heaviness and stiffness. This is often accompanied by swelling and a sensation of cold in the affected area.
○ Common Areas Affected: Typically in the lower back, knees, or extremities where Yang deficiency and Damp-Cold can accumulate.
3. Yin Deficiency with Heat:
○ Pain Characteristics: Pain can be sharp, burning, or hot, with a feeling of heat in the affected area. This is often accompanied by dryness or a feeling of restlessness.
○ Common Areas Affected: Often affects the back, joints, or limbs, with symptoms of heat and dryness. Pain may worsen at night or with excessive heat.
4. Spleen Qi Deficiency with Damp-Phlegm Accumulation:
○ Pain Characteristics: Pain is often accompanied by a feeling of heaviness and swelling. It might be dull and diffuse, and may include sensations of congestion or pressure.
○ Common Areas Affected: Common in the lower limbs, abdomen, or areas where Damp-Phlegm can build up.
5. Liver Qi Stagnation with Liver Blood Deficiency:
○ Pain Characteristics: Pain can be related to Qi stagnation, causing a feeling of distention or tightness, combined with dull, aching pain from Blood deficiency. This may also include emotional stress or irritability.
○ Common Areas Affected: Often affects the ribs, chest, or abdomen, and can also involve the head (e.g., tension headaches).
